• AMP Stack (Which versions of Apache, MySQL, and PHP are you using?)[/*:m]
• Network Environment (Are you accessing it locally, across a LAN, or across a WAN? Are there any NAT routers or firewalls?)[/*:m]
• What version of OpenFISMA, and how did you obtain it? (Did you download from SourceForge, did you check it out from Subversion? If a checkout, which branch or tag did you check out?)[/*:m]
• Steps To Reproduce (How do you reliably reproduce this error?)[/*:m]
• Expected and actual results (What did you expect to happen? What actually happened?)[/*:m]
